| 2012 Summer Registration and Payment
Information Registration:
You can register online, by mail, by fax or by email. To register
online simply click on the online registration link provided to
complete and submit your application. To register by mail, fax or
email, download the Microsoft Word registration form to your
computer, fill in the required information and submit it
appropriately. If you choose to email it back, don't forget to click
"save" on your computer before sending as an attachment. If you do not
save your document first, you will be sending a blank registration
form. If you have any problems or need to make alternate arrangements

Payment:
Once your player registration has been approved, you can click on the
appropriate following links to send your payment online securely through Paypal!
Option 1) Player full payment *one time
payment of $240 for the 2012 summer session
Option 2) Player payment plan *an initial deposit of
$100 and two monthly supplementary payments of $70 ea. Option
1) Goalie full payment *one time
payment of $120 for the 2012 summer session
Option 2) Goalie payment plan *an initial deposit of
$50 and two monthly supplementary payments of $35 ea.
*IMPORTANT* If you choose to use the "payment plan" options,
your deposit is due no later than May 1st, 2012 with 2 additional
supplementary payments due no later than June 1st, 2012 and July 1st,
2012 respectively. If you choose the full payment option, payment is
due by May 1st, 2012. If you
